Salvage Radiosurgery for Recurrent Cardiac Sarcoma: A Case Report

Primary cardiac sarcoma is a rare malignant tumor that arises from the myocardium. Surgical resection standard of care, and median survival ranges 6 to 12 months. The role salvage chemotherapy radiation not well defined. A 53-year-old female presented with acute congestive heart failure underwent complete surgical an undifferentiated pleomorphic left atrium, followed by six cycles adjuvant doxorubicin/hydroxydaunorubicin ifosfamide. An MRI scan demonstrated asymptomatic, 24 mm, recurrent atrial mass. patient was treated frameless robotic therapy over three weeks. dose 72 Gy in 15 fractions 84% isodose line. repeat at four weeks showed in-field local progression greater protrusion into atrium invasion ventricle. therefore elected proceed single-fraction radiosurgery. 25 one fraction prescribed 76% She tolerated treatment without any toxicity subsequently variety regimens, including tyrosine kinase inhibitors (TKIs) immunotherapy. Unfortunately, relapsed metastases spine pelvis. palliative multiple bony sites partial response. resumed TKIs but passed away due septic shock evidence failure. Fractionated SBRT ineffective controlling our patient's sarcoma. Our control disease months after radiosurgery toxicity. High-dose reasonable option for long-term unresectable sarcomas.
